Precision Dental Plans Discount Plan
Don’t worry about waiting periods, annual maximums, exclusions, or submitting claims. You receive the services you want, when you want them, at a discounted rate. You and your family may now receive the highest quality dental care at a reduced price.
Effective as soon as we receive your enrollment form and payment for you and your dependants. Dependants age limitations apply. Dependants are eligible until their 19th birthday unless full time student status is received. Full time students are covered until their 30th birthday.
Plan enrollment period is a minimum of six months. Plan premiums are automatically deducted from the specified account on the date that the policy was effective on a semi-annual or annual basis. If policy premiums are declined or returned not payable by your bank, your policy will automatically be canceled. To re-enroll you must fill out the forms again, provide semi-annual or annual payment and pay the ten dollar enrollment fee again. You may choose to opt out of the automatic renewal. Please contact a representative to fill out the necessary information.
All dental services are provided at a discounted rate. All offices on the discount plan offer discounts at different rates. Please contact the office of choice to get their contracted discounts. Listed below are average prices. These are not definite prices.
This plan is not dental insurance. Enrolling in this program gives you the opportunity to obtain the LOW FEES and at the same time, provide you with the basic preventive dental treatments.
We welcome children and recommend that their dental treatment begin early. Remember, prevention is less expensive than treatment. Regular visits for routine care are good planning.
This program is especially for the enrollee(s) and not assignable to another person. The patient is responsible for scheduling their two cleanings and X-ray appointment. Reserved appointments are required, except for emergency visits. There is a $25 fee for broken appointments when there is less than 48-hours notice.
The Plan enrollment fee and monthly payments are nonrefundable and nontransferable.
